Customers must have two forms of ID, one of which must be a valid photo ID. Military member must accompany family members to their appointments.
Please make an appointment for each individual. No double booking.
For UTANG members - If CAC is lost or stolen, you must fill out lost or stolen memo and get it signed by your commander. Hair and facial hair must be in regs.
